Beneficiaries
Beneficiaries can be designated using the DPMA Beneficiary Form. Complete forms should be submitted to membership@dpma.org.
If a member has not designated a beneficiary, DPMA shall name beneficiaries in the following order of priority:
- The Member’s spouse;
- If the Member has no surviving spouse, the Member’s children (including adopted children) or their descendants per stirpes;
- If the Member has no surviving spouse or lineal descendants, the Member’s parents;
- If the Member has no surviving spouse, lineal descendants or parents, the Member’s sibilings; and
- If the Member has no surviving spouse, lineal descendants, parents, or siblings, the Member’s estate.

To assist a Member’s beneficiary, DPMA has created a Guide for Survivors. This resource is intended to assist a Member’s beneficiary in making initial contact with organizations in charge of providing benefits to Delta Pilots. A DPMA representative is also available to help with questions.

Update Your Beneficiary
To update your Survivor Benefit Beneficiary, submit a completed Beneficiary Update Form to Membership@DPMA.org or fax it to (404) 559-9817.
EMPLOYEE SERVICE CENTER
Please understand that you should contact Delta within 30 days at the Employee Service Center at ESC.Delta@Delta.com or by calling 800‐MY DELTA (800 693‐3582) to request one of the following forms:
- Family Status Change Form (For Spouse or Ex‐Spouse), or
- Domestic Partner Family Status Change Form.
Please be aware you will be required to supply supporting documentation as appropriate to include Certificates of Death, Birth, and Marriage.
LIFE INSURANCE
Delta Air Lines has purchased a Term Life Insurance for the pilot as a benefit of employment negotiated through the Pilot Working Agreement. Contact Delta at 800 MY‐ DELTA. Please make sure to inform them that this is for a Pilot benefit. ALPA offers members Term and Accidental Life Insurance products.
